Patient Portal – We recently switched records systems to Providence’s Epic (CGFM is still an independent organization) and our old patient portal is no longer available. Please ask us to sign you up for MyChart to access your test results, after visit summaries, and billing information.
Mental Health Support – Times have been stressful. You can receive mental and emotional support on Safe + Strong or you can call the Helpline at 1-800-923-HELP (4357). The line offers 24-7 emotional support and resource referral not only to those experiencing a crisis, but also to anyone who needs it.
Video Visits – Many appointments are now by video using Zoom. Prior to your scheduled video visit a medical assistant will call you to guide you through the process. You will need to have a smartphone or computer with video functionality.
